I acknowledge that I have answered the above questions completely and honestly. I acknowledge that I have read the details of the training and FAQs in detail. I understand that by filling out this application, I am not guaranteed acceptance into the training program. I acknowledge that I have read the refund and cancellation agreement policy and that the deposit is nonrefundable. If Fit2Shine Studio cancels the training, I will receive a credit or full refund. I understand that once I am accepted, I am committing to 100% attendance. Any emergency situations will be discussed with the studio and training leaders. I have read and understood the attendance agreement. I understand that $300 deposit is due at the time of registration and if accepted, the deposit is nonrefundable.
